Acetaminophen does not dissolve (much) in water. If a powder of acetaminophen and something soluble were added to water and mixed for 5 or 10 min, and especially if you added some crushed ice to the mixture halfway through, the acetaminophen would be solid and easily filtered out by pouring the mixture through a coffee filter, or even just a t-shirt if you didn't want to wait as long, into another container.

Acetaminophen doesn't put that much of a strain on a liver.
