If you actually have peritoneal symptoms then you need an abdominal/pelvis CT or at the very least an upright abdominal xray, but more than likely because of your age you'd get an ultrasound.

That being said, peritoneal symptoms are abdominal rigidity, rebound tenderness, and over exquisite tenderness throughout. And it likely wouldn't improve with an enema. Also, you said the initial pain was left sided so what makes you think because you had reccurrent similar pain daily after working out it's appendicitis? If it actually perfed you wouldn't just be walking around, especially a day later. It would more likely be a hernia, But it also somewhat sounds like constipation/irritable bowel syndrome, but these likely wouldn't just start all of a sudden either,

Yeah my bad, I reread it and was correcting it but missed that part.

That being said, if it were appendicitis, that had perforated, you'd be toxic and feel like utter shit. And your symptoms wouldn't improve with a BM.

If all of your blood work was normal and your temp was normal, they likely would not even entertain the idea of appendicitis in general, especially with the history you gave.

Those are not the symptoms of appendicitis, seems like bowel obstruction or twisting of intestines causing ischemia and necrosis.. that’s why he felt temporarily better after an enema when fluid filled his lower colon

Might be an ischemic vowel issue

I know I know it doesn’t fit 100% but it has other symptoms, nausea, pain started after intense exercise (which I’m assuming involved intense ab exercises and twisting and turning of body), gases can’t pass through body, enema makes it better but it gets worse again because the main issue hasn’t been treated yet

Eventually comes in with peritonitis and right sided abdominal pain, necrosis fucking up the right colon and other nearby structures..

It’s twisting of colon, causing ischemia, which has turned necrotic and now leaking colon contents into the peritonium.

Other than some annoying teeny tiny symptoms, the rest of it kinda fits

The regenerative nodules aren't in response to inflammation, they are in response to literal lack of liver due to it being dead and scarred over. It's one of the only organs that can regenerate in such a way and if it didn't people would be in liver failure much easier.

It's a mixed field. You do procedures but you're also a clinician.

Physiology and pharmacology are your best friends and you need to know those inside out because of med interactions. Then, the ICU is a fucking war zone and requires fast response, the right medication and accurate dosages. The research you do it's extremely important, can be on burn victims, polytrauma patients, comatose patients, etc.

Lets focus on the latter, omega 3 FAs have been shown signifcantly increase testosterone levels in men after 3 months.
Physically strenuous activity especially high intensity ewxercise with legs also have a testosterone boosting effect.

It's not that complex. Most processes are similar enough to general principles that you can figure out how things work fairly easily if you understand the basics. The hardest part is the pharmacology but with newer technology you can easily see what meds are used for what, what dosages, and what side effects you can expect. Common things are common though, so you use typical medications for typical illnesses. In medicine most of the patients being admitted to the hospital are having congestive heart failure exacerbations, COPD exacerbations, basic infection like UTI/pneumonia, and diabetes complications.

You just get a grasp of the basics, then specialize on something or you become an all rounder, basically that's it.

Since i do the headaches went away, the thjng is the amount I have to drink is insane, it's been two days now the first one i drink beetween 10 and 15 liters, the second about the same now i am at the third and 3 hours after waking up i already drank almost 2 liters.
I thought it ciuld have been diabete (though not a severe form) so I stopped sugar but I ve read that drinking too much was unhealthy.
How long do you think it will take for my body to rehydrate? Should i go to the hospital (would want to avoid since the prices are insane?

Sure, there are downsides: if you don't have an emergency that will kill you in the next two months you can expect to wait some time before getting your test done but I think it's an acceptable deal knowing that the moment I should have a tumor I would have all exams and surgery done in three weeks top with no debt to the hospital.
Also, you can always pay up, get everything done by privates and be done in a day if you are in a hurry, nothing stops you; consider that Americans have a 6 years lower life expectancy than Europeans for a reason

No offense, but we all make fun of patients like you. We all have issues, we all have worries and concerns, just man the fuck up and deal with it and stop letting it control you. Very few people actually being treated with meds really need them, and up until recently, most people didn't have them.

That being said, SSRIs can take up to 4 weeks to work, though some experience it in 2. I would avoid the shit if I were you, it's not worth fucking with your brain chemistry.

One of the real tests you could do that doctors did back in the day is actually taste your urine when you aren't drinking tons of water, if it's sweet, you are diabetic. Though if it isn't sweet I wouldn't take it as a true negative.

Could still be type 1 diabetes, it is an autoimmune disease and most people are normal weight. You will need, at the very least, to get a basic metabolic panel which should give some answers. If it doesn't then they may need to go a little further with testing and check an A1c, urine sodium (along with serum sodium) and a TSH.

Interesting. Well you should see your PCP due to your BMI alone. But you should also check your caloric intake, because if it's 1800+, you may have a thyroid issue.

Here are my thoughts:

Those regions you marked match with colon, the middle being ileocolic (which gets a wave of peristalsis when you put food in your stomach), the left is ascending colon, and the right is descending colon. I believe your pain is related to colonic dysfunction of some sort.

Constipation is the most likely cause, but generally those stools are harder to pass. Spasms are pretty common with constipation though and you do have infrequent bowel movements. You could try taking something like miralax or magnesium. However, this wouldn't give an answer as to why your BMI is so low.

Thyroid issues can cause a wide array of issues in the body, and intestinal issues are a possibility. If your caloric intake is high and your weight is still that low, you may have a thyroid issue and you'd test that by checking a TSH and free T4/T3.

It might be worth checking a complete metabolic panel to evaluate your general organ function, electrolytes, and an albumin level as well.

As for the weight, it's probably just that the calorie intake is too low. Probably not the thyroid or else there'd be something else missing. It's most likely just the constipations then since I have had a colonoscopy before because of blood on my stool (red hot lying on the stool, not bigger than 3 Drops max) and nothing came out of it, no hemorrhoids, no cancer, nothing. My memory and vocabulary feel deranged certainly.
Is it just neurosis and depression? Have i been lobotomized(tm)?

The last time ive felt so upset about a MD was when i took roaccutane to deal with my acne conglobata in high school and the practitioner forgot to note that he doubled my dosage 6 weeks into the therapy overdosing me by 150/100 approx. Sorry for the rant.

Conclusion: Patients with pelvic fractures experience a reduction of their HRQoL, especially in the first 3 months. The HRQoL recovery continues to improve up to 12 months after trauma, and most patients do not achieve their pre-injury state of HRQoL. Prognostic factors for decreased HRQoL after pelvic trauma are a low pre-injury score, high ISS and female gender. A longer follow-up is needed to examine the HRQoL of pelvic fracture patients. We do not recommend the use of MAHS and MPS in the mid- and long-term follow-up of pelvic fractures.

If you’re providing anesthesia to critical patients working in a trauma 1, then you better know the caveats, pearls, and best practices in anesthesia reference texts for each surgery specialty.

If you want to supervise a bunch of CRNA’s in an outpatient surgery clinic, or just do day surgeries in them, then you really only need to maintain knowledge in common general anesthetic plans and maintain basic ultrasound skill to do blocks and spinals.

Albumin can be low due to lack of proper nutrition, that's all.

Excessive foam while peeing in association to albumin can be a sign of what's known as nephrotic syndrome, but honestly, if you had poor nutrition + nephrotic syndrome you'd probably be pretty sick right now.

Its probably pericoronitis.
If you can make 3% hydrogen peroxide solution,buy a syringe and a needle,try to wash the pocket that the tooth is forming with the soft tissue.
Pocket as a literally pocket on your jeans.
Tissue forms like that around the tooth.
In few washes and a day or two the pain should go down.
You can bend the needle to better fit between the soft tissue and the tooth.

If it foams a little dont be afraid it should do that.
Try to not inject in soft tissue.
Do not swallow.
After washing leave it for a minute or two to kill off bacteria.
Use common sense.
Get professional help.
