The reason why are countries waging proxy wars instead of real wars is because someone has such big ass nuke which can evaporate whole Earth.

There is no official information, I think.

It doesn't matter, you can always detonate many nukes at the same time, we are talking about the order of GTs rn.

I think as the megatonnage increases by the cube the surface blast area increases by the square. So a shotgun blast of multiple smaller warheads is more efficient at spreading damage over an area than one big one. Big ones are only targeted at heavily fortified targets where concentrating a lot of damage in a small area is important. Also old icbm guidance systems weren't as accurate so it used to be important to have a single warhead with a large footprint.

It's not an issue of building it, it's an issue of deploying it. The Russians have the record for the largest nuke, but even with a specialized bomber the shock wave from detonation made the plane disassemble.
