Is it really expected in the next 20 years though? I figure it's quite tricky to automate various complex brain and spinal surgeries.

wrong. newer and better tools might come in the next 20 years but it will always be a human operating these machines. every surgery is different from the other and sometimes you need to make judgment calls.
