All those health issues with spaceflight are a consequence of two things: zero gravity and radiation. Simplest brute force solution is to have the ship rotate (~2 RPM at diameter of ~400 meters) and to have several tons per square meter of shielding around the habitat (made from water, fuel, plastic or soil..). Environment inside such ship would be exactly the same as on Earth.
