Tom deserves the double lung transplant. He was born with his disease and will outlive the boomers. Dewi's likely to die anyway even without the pulmonary fibrosis due to his age. Sue is a difficult case to answer. She is a mom and her death will affect her children's lives. So you could make a substantial argument for her. But I am partial to Tom because cystic fibrosis is a shitty disease. The child had no say in his life. However, I will acknowledge that I am being very naive in choosing Tom.

Instead, we should figure out how to make artificial organs, so that we don't have to choose between people and everyone can live.

Yes but Tom's parents could have undergone 3 miscarriages before having Tom. So we don't know how Tom's death could affect his family. Women being mothers doesn't make them more special than men being fathers.

Is this your homework or something? Obviously little Tommy gets the lungs because he's the youngest and healthiest and therefore least likely to reject and you don't split up a pair of lungs if somebody needs a double lung transplant.

Giving it to Sue would save her, who still has 40 years to live and it would save the upbringing of her children which will affect 4 whole lives. She should get one (I hope she's not a Mary Sue)

Tom still has 80 years to live, but giving it to him will prevent Sue from having it. He will not get it.

Dewi will die soon anyway but if we have one left over, he might as well get one.

Sue and Dewi. 2 lives saved and their immediate families saved are better than one child who makes no income.

Just because Sue smoked out her lungs does not mean we should deny her health care. Just as Dewi is old does not mean we should discriminate against age (although successful surgeries in this age are rare). As a first-world society we should control our population and that means preserving existing life.

That's a very good argument, however assuming his parents are 30 years old, and we weigh psychological trauma with a factor of n, saving Tom would mean 80 + 180n (assuming he has a sibling) of years saved. Saving Sue would mean 40 + 360n (assuming she has a partner) years saved. Therefore, as long as we value actual years of life up to 4.5 times as much as years without psychological trauma, saving Sue would be preferable. Weighing actual life relatively even more than that would mean saving Tom.

Also, according to Wikipedia, cystic fibrosis is genetic, so Tom should be euthanized either way

Quality-adjusted life years would suggest Sue. Both she and Tom probably have like 25-30 more years, but if she's not smoking anymore those will be higher quality than a kid who will wind up with two more bad lungs. Also Tom should move to the US where he can get treatment for his CF and not need a double lung transplant at 8 years.
