Worse than the actual bubonic plague because antibiotics wouldn't work, and it would mutate more rapidly. Keep in mind that the bubonic plague today has a 10% mortality rate even with medical treatment, and if it were to be a virus instead it would be significantly harder to get rid of in the body, it kind of speaks for itself.
