I get 4.11 MV but It says the answer is 5.54MV what am I missing?

The question is retarded. A single point doesn't have a voltage.
A voltage is always the difference of the electric potential two different points.

It's asking for the voltage at v with reference to a ground point infinitely far away. ei. 1/r = 0

The question is fine. My quick and dirty calc comes out closer to Op than given answer, though.

Maybe the voltage spreads in decibels, bi-directionaly, because flat surfaces don't exist. You could apply line or a surace to point, but problem is for transfering voltage you need to have something that have it in space, therefore you need a surface covering it's object in direction. Then from point as ball voltage is spread to two points creating junction and jacobs ladder actually, which means volatage is higher.
