Bill is working off a flawed premise. No, not that he can euthanize a bunch of people. Gates and people like him believe if they reduce infant mortality, third worlders will have less children overall. If they give sex ed to everyone and allow girls to attend elementary school, that they'll have less children. If standards of living are increased, third worlders will act like first worlder and stop having children.
They believe this because social experiments they've conducted in primarily African countries show that it happens. There's one huge problem though. Once the NGOs who are running the experiment leave, the behavior of these people returns to baseline. Even with universal access to birth control, if there is no NGO around to nag them to use it, they don't. Even if they're given vaccinations and medicines that dramatically reduce infant mortality, they still pump out kids. The only way Gates vision works is if every third world village has a permanent NGO presence there. In that case, his plan might work though we can't be absolutely sure as the experiments have only been run short term.
