The actual antipsychotic drug/molecule is transformed into a decanoate derivative of the parent molecule (the one that can is taken orally) so it can be more easily dissolved in the carrier oily solution that you are going to put inside you. The drug slowly diffuses away from this depot to your blood, where it is metabolized back to the parent molecule again by esterases enzymes. The decanoate derivative can’t cross the blood brain barrier because “it’s too big” of a molecule
