In juvenile idiopathic scoliosis, (OP pic), the one which starts in a healthy kid around puberty or early teens, something goes wrong with the neurological control mechanisms of spinal posture. Nobody knows why, or really how to treat it. There are some systems, Schroth, for example, which do get good results, but it's a combination of exercise and bracing, without understanding the underlying mechanism.
Over time the bones of the spine, particularly the angle of orientation of the facet joints, change shape and the scoliosis becomes more established.
In general terms, i.e. not juvenile idiopathic scoliosis, nobody is symmetrical. We all have slight asymmetries and imbalances. Chiropractors seem fond of telling people that their slight asymmetries are scolioses and they need to get many treatments from them to get fixed. Which is almost always bollocks. Many bodies work well with these asymmetries.
