That Italy is so incompetent that they are undertesting on such a massive scale that out of several hundreds of thousands who are infected, they only detected 10% of it. Another possible scenario is that it's a mixture of the two most likely cases, that both Italy is having a lot more deaths because of it's population age and because they are undertesting, making the death rate seem unnaturally high. There's also the issue of news saying that certain hospitals aren't treating old people at all, using war protocols where only people with highest chance of surviving are being treated.
Quite honestly there was a specialist who predicted this whole mess who said that in general even China wasn't detecting most of the cases, that was before SK started doing the shitload of tests.
