Well there's basically always sterics isn't there?
So it just indicates that it's particularly strong?

Yes, it's indicative that the R-groups are limited/sensitive to size. This notation is to essentially tell others who are looking to perhaps synthesize this piece or suppose a connecting R group to mind their goddamn space and don't start a rant on pubchem how it doesn't work when you're trying to shove two aromatics right next to each other fuckwads.

it refers to a steric clash
i.e. the two R groups are bulky, and due to being axial on the chair they bump into eachother which is unfavourable. Generally you want those R groups to be equatorial where possible.
