We know the underlying disorder in this case as well, gender dysphoria. It is logically inconsistent to treat gender dysphoria different to, e.g., schizophrenia.
However, as opposed to treating it like any other mental disorder, we allow patients to self-harm (perform the acts we consider a symptom of the mental disorder), so long as they're properly informed. I fail to see how that can be interpreted as anything other than unethical and in conflict with the concept of "do no harm."
