My dad (who is a doctor) told me that in his experience, people who get really fucked up by something (trauma, disease, whatever) roughly form two categories: people who make a fuss, and people who go quiet. The people who just listlessly lay down quietly tend to be the most fucked. If someone goes from loud to quiet, be worried.
That being said, the 2nd most painful of experience of my life (multiple leg breaks, but no head trauma), made me go into a quiet shock even before the pain really hit.
But during my most painful experience by FAR (muscle spasms in neck), I literally could not stop involuntarily screaming. So obviously it's a little more complicated than reaching some threshold of pain.
