They're more likely to smoke, eat trash, not exercise, never go to a doctor, and do dumb shit that kills them. If you're not a retard then you'll enjoy much cleaner air in your environment by living outside a city. You'll also enjoy the lack of constant road noise. But if you want to live in a rural area where your neighbors are intelligent, then expect to pay for it. It's completely worth it. Only downside is you'll likely have to drive half an hour to get a good restaurant.
