But if you operate under the assumption that dilaters literally are who they think they are then and it's not a mental illness then the only thing to do is let them do whatever the fuck they want. You're only supposed to treat an illness, and being a dilater isn't considered an illness anymore.
Of course the distress they all experience of not fully expressing how they feel is a mental illness. And of course the recommended way to treat that distress is to make life changing cosmetic decisions. It's literally a semantic game they played to get their way.
