If you want a life and decent money, the only option in surgery is ortho. As a general surgeon in private practice, I need to perform x amount of surgeries to be deemed proficient from hospital medical boards for privileges and take several days of 24 hour call. Those call days I get zero sleep, the ED always calls to “borrow a set of eyes” on this case or to consult on another.
I probably spend 30 hours a week actually performing surgery, 10+ rounding / attending, and 20 hours per week at clinic following discharged patients or patients pursuing outpatient surgery.
Tack on call, reimbursement rates, and you’ll find OP is exactly right. You barely break even on some cases, and we are highly litigated.
I’m not complaining, just wanted to validate what OP is experiencing is a true depiction of general surgery life. We, as a specialty, have the highest depression rate (per response poll, subject to response bias of course), highest amount of hours per week worked (followed by other primary care fields), and have the least job satisfaction per polling data.

Hang in there OP, consider becoming locum and just travel and take a few months break in between.
