Theoretically speaking, if a man were to copulate with the grand-daughter of his cousin, what are the chances of the child having birth defects? And, theoretically the man is 21 and his cousin's grand-daughter is 19. Would they have a similar genetic compatibility as 3rd cousins? Or am I seeing it wrongly?

This is out of curiousity. Theoretically, I think it's alright for people to copulate with their 1st cousins. The chance of birth defects is 3% for the average population and 6% for first cousins. The risk is greatly overexaggerated.

Let's ignore the incest angle and focus on biology for a second.

Inbreeding by itself isn't dangerous, it's the probability of your relatives sharing the same recessive genes you do that makes it dangerous. If you inherit a recessive gene for something dangerous and then marry outside of your family then the odds of them also having it are low. The closer someone gets to you on a genetic scale the more likely it is that they'll have it as well.

The risk of defects for reproducing with your cousin is less than ten percent. For the granddaughter of your cousin, you might as well expect nothing higher than normal because of how many other genes will be in there.

There's a large generational gap. My father is in his late 70s while his cousin managed to be a grandmother in the same time he had one child.

Yeah it gets to be a problem after several generations of inbreeding which allows defective genes to accumulate.
If it's only once every few otherwise outbred generations the risks aren't much different to normal.
And if you have good genetic stock inbreeding might reduce your chances of birth defects.

Even having a baby with your first cousin has very minimal risks for birth defects. If it's your cousin's granddaughter there would be very little difference (if any) compared to rate of birth defects between two strangers.

where did you get those numbers? last I heard it was ~2.5% for unrelated and ~3.5% for 1st cousins, based on a study comparing rates of birth defects between Brits and Pakis (20-30% of Pakis are born to first cousins)
