If it's about perceiving things that aren't actually in front of you, i.e schizophrenia, then you're insane
If it's about psychopathy, NEETdom, inceldom, trannydom, schizoidism and all the other things deemed a mental illness in modern culture, then you're not insane but merely not fitting what society finds as acceptable so as psychiatry remains with tied hands and unable to classify those as harmless differences in personal preference since doing so would upset the population and lead to psychiatry not existing anymore, they pretend that they're mental illnesses and the DSM swaps definitions around them with every next edition depending on what society finds acceptable
