Is it permanently science fiction? Is there a chance that it's possible? Which field is making strides towards achieving it?

Possible yes, but any civilization advanced enough to do it, would just bend space to form worm holes and create gateways from one area of space to another. Much much easier than teleportation.

Depends on your definition of "you". We're already ships of Theseus with the constant biological rebuilding and mental remapping. Actually, during the transmission you're not mutating at all since you'd be digital data with piles of error correction codes. For the first time in your life you'd be the same person for potentially multiple seconds.
