This is not a homework board. Nonetheless I will help because I'm bored.
Every point on a polar plot can be written as a cartesian point (x, y).
The distance of this point from the origin is described by r, and the angle from the x axis is described by $\theta<script type="math/tex"><br />This can be used to form right triangle where the hypotenuse is the length r and the angle of reference is <script type="math/tex">\theta<script type="math/tex">.<br />To get the (x, y) coordinate of each point, simply get the base and height of this triangle.<br />The base is rcos(<script type="math/tex">\theta$ ) and the height is rsin( $\theta$ ).
You should be able to complete the rest yourself now

Just replace
r = sqrt(x^2 + y^2)
sin(theta) = y/sqrt(x^2 + y^2)
and simplify.
