Because if it's ONLY the former, then you're operating from mistaken assumptions about how diseases work. Plenty of people suffer from chronic diseases without a genetic cause (or at least without any particular one known exclusively disease state determining genetic factor; you could always argue some combination of genetic factors created a vague set of circumstances that allowed for environmental causes to have more of an impact than they hypothetically would have otherwise).
There are correlations between the distance you live at from the equator and multiple sclerosis for example (thought to be due to sunlight intake and the vitamin D you would / wouldn't receive depending on that sunlight intake). And inflammatory bowel disease is often lumped in with the chronic diseases considered "diseases of affluence," which means it's seen more frequently in developed nations than in developing nations (thought to possibly have something to do with the lack of exposure to bacteria and/or parasites in developed nations with thorough and consistent sanitary practices).
