Not necessary for elementary QM, as long as you have a general idea of what it is and how it relates to HUP. Multivariable calc and linear algebra is plenty.

typically intro QM is taken in the the second year of undergrad, so by that time you should have taken (physics) intro physics, mechanics, and E&M and (math) the multivariable calculus course, the differential equations course, and many students take the linear/matrix algebra course at the same time as intro QM.

Yes, absolutely. The Fourier transform of a spatial wavefunction gives the momentum wave function.

You don't have to go into detail if you understand the basic concept. That goes for linear algebra too.

they require “thermal physics” which is the typical thermo/stat mech undergrad course. one semester of EM is standard. the second semester of quantum/advanced theory stuff/computational physics are probably options for the two required upper level courses. duke is a good school overall but not STEM heavy (not like MIT or Caltech) and this is typical of what you see at that type of school. i would guess that physics majors (not double majors) take probably 4 or more upper level courses (instead of the minimum 2) if they are aiming for physics grad school
