Some fantasy settings, especially D&D, have vast underground realms with immense caverns and cave systems big enough to hold entire cities and civilizations, like dwarves and dark elves. What sort of geological activity could form such large caves on an Earth-like planet? My best idea is massive ancient lava tubes like the ones on Mars. And how would a civilization adapt to such conditions?
