Not sure what the density of vacuum fluctuations is, but youd need an electron-positron pair right on the edge of it, and since most of these processes are happening on much much much larger scales I doubt it would happen within reasonable timespans.
But thats not even considering quantum effects which are inherently necessary for the vacuum fluctuations in the first place, and really since there is no quantum gravity theory there isnt a proper answer to any questions about black holes on that size.
iirc a black hole of that size might not even have a schwarzschild radius at all and just be a singularity floating around in the vast emptiness of its surroundings with nothing else in its area for like dozens of orders of magnitude...
