We can sympathize with him not on a personal level, but on a level of "Yeah, that's probably what an alien would think/do with humans".
Anno of course stated he wanted to portray Kaworu as very good and perfect, however if individual viewers wish to interpret him otherwise they can't be blamed for that either. It's just their opinion.

There's no reason for us to assume he lied there, his inner thoughts confirm that to. It is a true smile, both to Shinji and to Rei.
We don't need to know about his loneliness, he's already fleshed out enough in the episode. Arismael when talking to Rei didn't even know what loneliness was, likewise Kaworu may be lonely but doesn't even realize that. He wished to make a connection with Shinji and learn to understand him anyway, so we can surmise just from that he may be lonely. We don't need NGE2 to tell us that.
NGE2 is like an extraneous bit of fanservice for fans who wish to see their favorite characters in different situations. It shows "what if" situations. You don't need to rely on it to understand Kaworu, you can understand him just well enough in the episode by listening to what he says.
Certainly if Shinji showed him the joy in living things would be different, Kaworu himself could not comprehend why lilin wanted to live so much. He sees life and death as the same thing, so why not let the humans live if they value it more than him? It's a very alien-like way of thinking, something that we humans can't really relate to, and that's why Kaworu is such a brilliant character. He is genuinely otherworldly.
