It varies from person to person like any other fetish. People with a ballbusting fetish can be full sadists, masochists, or both.
Personally, I just like how the testicles is what makes a man who he is, contributing in giving him his strength, yet it's also one of his biggest weaknesses. That alone is just very erotic.
