I can totally see why you'd think that it's his second load, but it really says he's gonna have his second kid (? = give birth to ?? = second ? = fetus). Unless it's a euphemism for blowing his second load, lol. Too bad I can't ask any of my native speaker friends what they make of that sentence, since it's porn. I did think it's a little weird he's saying that in the middle of sex, but since his being a father is part of the plot, I still think it's plausible.
