The act where a character fucks himself in that way is called "selfcest". You can take picture attached as example - Genji, cyborg character, fucks the guy he once was, before most of his body was replaced with robotic parts.
The other question is - why? Well, there's more than one answer.
1. People find that person so hot that feel that there should be more than one of him to exist to achieve the satisfaction
2. It's part of the plot - it might be some kind of scientific experimentation, clone, time-travelling or a rip in a multi-universe continuum
3. It's to show the changes and flow of the time, it might be nostalgic
4. Whatever, it's just porn for us to enjoy.
