What is not simple is finding out where the point of failure is and how to act on it. It could be hearing loss because you blasted too many db through your ears, a damaged nerve, an inflammation, or absolutely anything from source to destination.

When there's a lack of input the brain will add noise.
When the signal is disturbed by other interferences, the brain will attempt to hide them as best it can, but past some point there's not much it can do.
