When you get a Ph.D does everybody have to refer to you as "doctor" everywhere you go? Even when not at work?

Only people who care. Im a masterslet who has accepted never obtaining a PhD, and I refer to all math PhDs as "doctor". Don't respect or care enough about other fields though.

When at work it would feel silly.
But particularly in Germany if you have to deal with an annoying person, let's say
a police officer, it is nice that you can force them to use your title. In my case
they'd have to address me with Dr. Dr. before my actual name.

The only people I've ever seen use doctor are the new PhDs. Senior professors don't even want you to call them by their last name.

Same in Czech Republic, I guess its the old Austrian influence. Not just phds, either. People will say 'Mr Engineer' if you have the equivalent of an MSc. They even do it with jobs. Not just 'Mr Doctor', but also 'Mr Lawyer', or 'Mr Director' when you're speaking to the guy in charge of a company.

Every gender studies professor always insists on "I earned this, you'll call me Doctor blah-blah", but math/physics/engineering Drs don't care
