Why do doctors tell you to fast 12-18 hrs before a cholesterol test?

The blood bank I donate to does cholesterol screening and they tell you to eat a meal before donating. So you can compare their results to the labs your doctor orders to get your range.

Is that true? I've been fasting regularly for some time now and consistently get really high cholesterol results despite being skinny as hell. (I don't take statins yet because I was worried taking pills daily in my 20's my affect me longterm and I wanted to leave it for later.) I guess I'll look more into this and try a non-fasted test, usually I don't eat for at least 20 hours before one.

Are you doing time-restricted feeding? Try three (healthy) square meals a day, for a few months, and get your lipids retested.

It's not for the LDL, you can eat as long as you only are looking at the LDL. Fasting is there so that the TGs aren't deranged. LDL also will not increase significantly for the amount of time you fast. Moreover it is likely that the "normal ranges" that determine whether your LDL is high or not is likely based on a population of normal people who did the blood test fasting, so it wouldn't make much sense to not fast. Lastly, a good doctor should not just prescribe statins for anyone whose LDL number is a little bit elevated. If the statin is for primary prevention of coronary events, all the other risk factors should be taken into account before starting a statin.

An increase of LDL of 0.5 is very unlikely to be the difference between prescribing vs not-prescribing a statin.
