70% of Americans (really western world) are fat or overweight. So by large amount, almost everyone you see is a fatty in American eye, thus the fat normalization occurs.
On top of this, ~1/3 of Americans are morbidly obese, thus there's a huge percentage of people who are beyond saving and those become normalized.
