I just start the exam in the reading time, skipping questions for which the answer doesn't immediately come to me. That way when writing time starts I've usually already completed a good 25-40% of the questions. It's also unwise to do questions that will require a lot of pen work to transfer the answers from the scrap paper. If it's a math exam, or one that requires you to do problems, it is a good idea to explicitly train for speed in your study routine. Get used to trying to go as fast as you can go without fucking it up during your practise sessions, and you'll find yourself getting faster both as you become more familiar with certain types of questions, and at planning your attack for problems in general.
