In europe it's almost guaranteed.
In the US only if you're a woman and your supervisor is attracted to you or you're a "prodigy" child and universities basically throw all their rules into the trash for you.

How long your PhD takes isn't so much about your thesis work as it as about how long your adviser wants to pay you $25k/yr to work on his grant. Usually, they want more than three years because it takes a long time to even get locked on to what is happening in the first place.
