Realistically speaking, it is unlikely. The only viruses that are near 100% lethality are rabies and HIV (untreated). Is it possible for a virus to essentially become another virus?

If we let the imagination go wild... Some of the deadliest things out there are prions and rare brainstem cancers (high lethality, because it's tough to cure it without damaging the stem). Perhaps a virus *could* evolve to produce prions or induce a brainstem cancer (sort of like HPV leading to cervical cancer).
