Plans are super nice, I develop tons of them then I end up here in this situation every fucking time because I gave up mid way through.

I have noticed that eating lots of high energy and nutritional foods helps me. Healthy burgers(ei good meat, fresh veggies on them), milkshakes, eggs. also hydrate a lot, and caffeinate. You can do it OP. I recently did this on finals (fall semester so like a month ago) and I did rly well. and just accept that it'll be hard. find a tv show or two thats rly comforting and watch that as regular study breaks(like 15 minutes on 15 minutes off for me). I was able to do this for 12 hours at a time at the high end

another impt factor was exercising 6 days a week ( abt 6 miles a day for me, but im a runner) like 30 mins of light exercise should do it id think

Caffiene, dicipline and healthy foods. Avoid alcohol. 3 weeks is alot, and if you are a masochist in some way like me, you can hold down 16 hours of study per day easily.
