There are two types of "altruism", one is dumb retarded "altruism" where people don't why they're doing what they're doing and are simply doing it. The other is people who have a deeper understanding of the world, the actions they take, the consequences of taking those actions, and acting on those to benefit the greater good.

When someone asks why he helped the homeless, the answer you get would be "because I saw him fall down" or "he looked hurt" or "I wanted to help." These are good and all, but these are basically "dumb" altruism. They have no effective power over any thing beyond the circumstances.
Another example: Bleeding heart liberals. They cry about income inequality, wanting wealth tax on rich to get rid of inequalities, wanting to shutdown gifted/talented programs because students are not diverse (not black), etc. These dumb altruism are a hindrance on large scale projects but work fine on small scale.

However if he ignored the homeless, the homeless would continue to suffer, hence 911 call.
Example two: Guy invests his life building out companies to help solve the problems of the world, he starts a profitable company that produces clean electric cars, cheap rockets for sciences, cheap energy production, yet he won't give out his money to homeless or the poor because he understands that you can only fix things when you have a successful biz
