You have a cavity. If left untreated it will infect the pulp which means you'll need it removed and ideally replaced with an implant; if it's this bad most dentists will suggest a root canal but don't fall for it, you'll have dead tooth in your mouth.

Depends which teeth and if implants follow (if so, then the face is fine). Wisdom tooth removal has the largest effect on face structure. Also Imagine not going to the dentist for 19 years. No wonder why you're in excruciating pain. Just get every single tooth removed, especially if you're past 35. Get implants and caps.

You can get wisdom teeth implants which will preserve face structure. They're pretty rare but still possible.

It's not like prehistoric people had a lot of options for what they ate or how they cleaned their teeth
The only reason teeth hurt is probably so people dont pull them out in retarded caveman rituals or some shit.

It is because your body packs every single area which needs to have fine-grain sensation and control with a fuckton of nerves. Your entire mouth, tongue, fingers and toes and probably some other obscure areas I dont remember now.

I think its because teeth evolved to be really good for feeling stuff - after all, that's how most species grab things - and as a consequence these nerves also transmit pain.
