Jokes aside, if you're really at a loss, here's a few ideas
- Open medical journals and look for mathy stuff. See if you can improve anything, do a cleaner proof of something or take some idea further.
- Contact people in the medical departments who work with mathy stuff. Ask them for ideas. High probability of success, if you're prepared to ask enough people.
- Look at epidemiology. There's tons of math there and you're very likely to find something interesting. Veritasium recently made a video about it, and how it relates to random walks, watch it for inspiration.
