Yes, it can dislodge and cause more issues. The coronary arteries are typically the only ones that get these procedures done. If they dislodge, they will go deeper into more narrow vessels, occluding downstream artery access, but won't get through the capillaries so it won't embolize like PEs. Angioplasty (using a balloon to squish the plaque down) doesn't usually have much of a risk of dislodging the lesion, but the other ones do. They make a basket that they can position downstream to pick up any material that could fall off, but the docs don't use it most of the time
