The congress of United States (Parliament in other countries) is what controls the state, they come up with all the changes.
President is legally obliged to sign and accept every change the congress proposes, and say that it was his idea every time.
Then, when things go terribly wrong, it's the president who gets all the backlash, often from the public and Congress (even though he had to sign it).
Now apply this knowledge to any democratic country. It explains a lot in USA's case.
