Second flight was because they of sloshing issues which wouldn't have been an issue if that was the only issue, but stage separation bumped the engine causing the TVC to malfunction thus it was unable to correct the oscillation from the sloshing fuel.
Third flight's issue was the residual fuel causing issues with separation.
All of those were tiny random unlucky chances imo. There really was a possibility that their first Falcon 1 could have launched properly if launched from Vandenberg.
