You don't. You allocate enough time and energy to be able study at normal pace then you take exams.

Even sitting down for 2 hours is difficult despite the fact that I could study for 12 hours straight when I was 17-18.

Is it just practice/getting used to it?

Yes
there was some info about doing something for 21 days (three weeks) if you manage to do it, it should become a habit, I started going to the park jogging and have done it for a month (restday every 3-4 days), after that going for a run just felt natural just like having to eat lunch, I think same method should apply to studies too
