This is incredibly infuriating, paying out of your ass for it and then further having to humiliate yourself like this, I feel for you.
Just write them you feel the obligation to become a physician, and that you've experienced what bad medical care can do to people due to a relative, or preferably non-white friend, of yours suffering some sort of lasting damage due to malpractice, and that you know that you can put the effort in to do it better etc. Tug at the heartstrings a little, it's mostly middle aged women reading these shitty essays anyways. Don't think about what you would want to hear from an applicant, think about what a pea-brained women past her 30s would want to hear

Tell them that you think public health is closely intertwined with democracy, and that you are interested in medicine because you belief healthy citizen are the first step towards a healthy society. Then as specific examples, mention the COVID epidemic and how a simple respiratory infection became first a public health problem, and then a global crisis. You can also bring up people's poor mental health today and point out that a lot of schizophrenics and crazies are behind the rise in conspiracy theories and white nationalist terrorism, and that you are motivated to help America solve it epidemic of mental illness and political extremism.

If your essay doesn't make clear how you have been discriminated against as a black woman you are not accepted.
In my country med school is so compromised, they just have different tests for males and females, so they can have their desired 60% female med students.
