Depends on the context, the sterile neutrino is the only one that can be a fundamental majorana fermion, but other neutrinos could be effective majorana fermions, and for some reason people don't seem to care about distinguishing between the two.

No, the mass would be due to the Higgs mechanism, but it will act like intrinsic mass if you don't look close enough. For a fundamental majorana fermion the mass would not be caused by something else and just be there.
