Is there a way to find out if I have diabetes without going to the doctor or having to prick my own finger? I've been having non stop thirst, dry mouth, headaches, and fatigue for the past week and I'm pretty sure it's diabetes, but I can't stand the sight of blood. Is there any kind of saliva or urine test I can do at home that will tell me if I have it?

we obv prescribe the for patients with DM2 and heart failure and it seems to work fine in reducing the rate of progress and the number of episodes. it really hasn't spread much to patients with just a heart failure yet, even though it seems to be good for that too. It's a bit pricy at the moment too.

You mean throw SSRIs at everyone? Psychiatry doesn't have cures and even then there's a ton of infighting because the fda is retarded and listening to them is hurr durr ssris (despite literature advising against this and blatant contraindications for patients with emotional blunting) and if a psychiatrist tries to think for themselves or use literature then hurr throwing darts at a wall. There's a ton of retarded psychiatrists like there are any physicians but outcomes with psychiatry are more contingent on the physician's judgment rather than following flowcharts

People just want someone to blame when they don't recover instantly

Depression at least is really hard to treat from what I know and ssris I think are only 60% effective or so, and even if they are effective I’ve had so many patients tell me they stop because of the side effects. Coming from a pharmacist perspective a lot of them get way too hung up on following guidelines and not trusting the prescriber’s clinical judgment, or their own for that matter. Psychiatry is so patient specific I feel you just have to throw guidelines out the window sometimes, they’re GUIDElines, not follow this or die lines.

The only value I find in SSRIs is quickly stabilizing a patient and that's mostly escitalopram. Studies accounting for placebo, bias, over-diagnosing depression etc. find little value in SSRIs and often the people who respond will respond to

Akathisia specifically is absolute hell and patients need to be aware of it as a symptom so they can drop a medication causing it immediately; if psychotic disorder then call me so I can figure something else out. It's often misdiagnosed as part of the disorder being treated so doses are doubled down on and oops. Seriously I was getting suicidal after an hour of that shit the two times it happened, even two months of severe effexor withdrawals had nothing on this.

Fuck even pindolol has antidepressant properties (5ht1a agonism) on top of the obvious sympatholytic anxiolysis. Probably forgetting a bunch

I’d like MAOIs more if they didn’t interact with so many foods, but I have heard some of the concerns may be overblown, many are still really afraid of them though. It’s a shame because they’re really effective for depression from what I’ve read. In any case I rarely see them in retail.

It is extremely overblown and they're empirically the best tolerated class. You basically can't eat blue cheese/drink yeasty (cloudy) beer and you're good, fuck-ups are almost always just transient discomfort and hypertensive crises are extremely rare.

They're not used often because dumbfuck psychiatrists fell for hysteria and never bothered thinking about revisiting it, there's tons of modern literature addressing the bullshit. If you're really afraid you can wait for RIMAs to hit the US but your hangups aren't any different than those of people afraid of SSRIS. I'd say try it, if you need meds in the first place then you need them. You can be overzealous about food at first but you can always just try small amounts and keep increasing it if you're worried, the nervousness about hypertensive crises fades quickly

I was also on parnate myself for a while and had no problems I can remember. I only got off it because I had to switch psychiatrists to an argumentative dumbfuck; earlier in the thread I blogged about a retard trying to give eme haloperidol as an adjunct, anyway rhag was him.

Alright I'm going to answer you seriously. Aside from acute risks of heart going oops, the problem with coke is largely what's causing the heart to rush. Hypoglycemia, chronic stress/ptsd efc. cause tachycardia on their own and while long term it's very not good for your heart it's not acutely dangerous
