Regardless of the comic source, in the film canon, Steve sees himself as a soldier regardless of the uniform he wears and the thing on his head is a helmet like a soldier is meant to wear. (#2. Even the helmet in #2 was just a prop for the USO showgirls. #3 is a certified, Howard-stark made work of art. 5, 6, 7 and 8 are likely all made by tony based on howard's #3.)
The only reason there is an eye mask with it is to clue people in to who he is. In TFA, a dumbass prisoner asked who he was and he actually had to say "I'm captain america" to gain the trust of the allies he rescued, but if he were in-full #1 costume, they would have put 2-and-2 together regardless of how silly it may have looked just because the movie showed soldiers reading the propaganda comics with him.
In the Avengers, the cops recognized the costume, but looked at him like he was some insane shmuck since cap was legally dead for 70 years in-universe to them. When THEY questioned him, he didn't need to say a word after and realized he was the real deal after he rekt the Chitauri squad.
So, in principle, in the MCU setting, the world is aware of captain america in the same way they are aware of uncle sam, but it's thanks to the propaganda, so there's no point in cap being seen "in action" without his helmet, specifically because he could still get his skull caved in rather easily in his line of work without it. It also legit commands respect.
