I'm pretty sure a lot of Stark's problem was also due to him not helping Rhodes out with suit upgrades. (for good reason IIRC but guilt doesn't care)
Overall people never sat down to think about how to handle Ulysses' visions, much like the Minority Report movie. Haven't read the story. Just the concept of getting eyes on events before
they start, stopping them before they happen and reacting appropriately seems beyond everyone.
Thanos? Use maximum force. Bank robbers? Stop them before they kick things off, hand off to police or threaten as appropriate. Guy about to shoot his wife? Stop him and get him into therapy.
Or whatever as long as there's a sound fundamental doctrine in place and thought is given to each deployment.
